Community Paramedicine

Union EMS Community Paramedicine is a patient-centered program designed to bring healthcare beyond the traditional emergency response setting and directly into the community. Our Community Paramedics work with patients, healthcare providers, and community partners to help address healthcare needs, support chronic disease management, improve access to resources, and reduce preventable emergency department visits. Our goal is simple: provide the right care, at the right time, in the right place.
