Schedule a Transport

Please complete the following forms and acknowledgement to request non-emergency transport. Users must acknowledge that they understand and agree to the following:

• This request is for a scheduled medical visit.

• I understand that all requests for non-emergency transport should be made at least 3 business days in advance.

• I understand that scheduled transports may be delayed or cancelled by Union EMS due to emergent needs.

  • Union EMS will contact you directly as soon as delays or cancellations are anticipated and work with you to reschedule the appointment/transport.
